Our experienced team made up of nurses, therapists, home health aides, medical social workers and support staff work together to support each patient’s recovery, rehabilitation and overall well-being. Skilled Nursing Care

Under the direction of your physician, our Registered Nurses (RNs) and Licensed Practical Nurses (LPNs) provide expert, in-home care that’s comparable to what you’d receive in a skilled nursing facility. Services include medication administration, wound care, patient education, caregiver support and care coordination – all designed to help you heal safely at home.

Physical Therapy

Our licensed physical therapists help patients regain strength, mobility and balance after illness, injury or surgery. Through customized exercise programs – sometimes with specialized equipment – they support recovery and improve safe movement at home.

Occupational Therapy

Our occupational therapists help patients regain independence in daily activities by improving fine motor skills, hand function and cognitive abilities. This care is especially beneficial for those recovering from stroke, neurological conditions or injury – all in the comfort of home.

Speech Therapy

Our licensed speech-language pathologists provide in-home care for speech, language and cognitive challenges, as well as swallowing and feeding difficulties. Therapy supports patients recovering from stroke, neurological conditions or illness, helping improve communication and quality of life.

Social Workers

Our medical social workers provide emotional support, counseling and resource coordination for patients and families facing life changes. They assist with financial concerns, care planning, and transitions to new living environments or support services.

Home Health Aides

Our trained home health aides assist with personal care tasks like bathing, dressing, grooming, and mobility—helping patients stay safe and comfortable at home. Working under the supervision of a nurse, aides also provide companionship and monitor for changes in health or condition.

Palliative Care

Palliative care offers relief from pain, symptoms and stress for those living with serious or advanced illnesses. Our team provides comfort-focused support, even when a cure isn’t possible, helping patients better manage their condition and maintain quality of life.

Telehealth

Our telehealth program uses remote monitoring technology to track vital signs and health trends from the comfort of home. Patients receive easy-to-use equipment, and our clinicians review data daily – alerting providers to any concerns. This proactive care helps prevent hospital readmissions and supports safer, more connected healing.

Disease Management

VNA Alliance’s Disease Management Program provides evidence-based care for patients living with chronic illnesses such as COPD, heart failure and diabetes. This structured approach helps standardize care, reduce hospitalizations and support better outcomes – during acute episodes and throughout home health recovery.

Patient & Family Education

Our clinicians provide personalized education in the home to help patients and their families better understand medical conditions, treatments, medications and daily care needs. By offering clear guidance and support, we empower families to feel more confident, informed and prepared.

Mental Health Nursing

Our skilled mental health nurses provide compassionate, in-home support for individuals experiencing emotional, behavioral, or psychological challenges. We focus on medication management, crisis prevention, emotional support and coordination with behavioral health providers.

Wound Care

Our team of wound care–certified nurses and physical therapists provide advanced treatment for surgical wounds, pressure injuries, diabetic ulcers and other complex conditions. Using evidence-based practices and personalized care plans, we promote healing, prevent complications and help patients recover safely and comfortably at home.

Our home health professionals help reduce hospitalizations by delivering timely, coordinated care onsite. A nurse is on call 24/7 for clinical guidance and urgent needs, and we see referred patients within 24–48 hours of physician orders.

VNA Alliance bills under Medicare Part A, which means no co-pays for eligible patients. Your residents receive skilled, compassionate care and your staff benefit from a reliable, experienced partner.
